THE ER CARD / BRACELETS

Vital medical information is accessible round the clock through the ER Card electronic Personal Health Record (ePHR) service. Have peace of mind knowing wherever you are, your most crucial information can be immediately accessed.

When care is needed in an emergency or on a routine basis, medical professionals and care providers have immediate and secure access to medical information such as:

• Medications
• Blood type • Allergies to food or medication
• Medical conditions
• Emergency contacts
• Physical or verbal limitations
• Major surgeries
• Primary care doctor
• Treatment specialists
• Emergency evacuation information
• Advance directives

The ER Card improves communication between patients and caregivers, resulting in the delivery of the most effective treatment, which sharply reduces medical mistakes. The ER Card enables people to take charge of their care and stay safe and independent in their homes.

In addition, clients:
• Enroll with assistance from care managers and customer service support.
• Have access to their records through the internet, telephone or fax from their home, hospital or doctor’s office.
• Receive care reminders and follow-up within 24 hours of emergencies.
• Get special support if they have intellectual, physical or verbal limitations.
• Receive referrals for community-based services and interventions
• Have advance medical directives and test results attached to their electronic personal health records.

THIS PLAN IS NOT INSURANCE. This plan does not make payments directly to the providers of medical services. The plan member is obligated to pay for all healthcare services but will receive a discount from those healthcare providers who have contracted with the discount plan organization. This plan provides discounts at certain healthcare providers for medical services.

This discount card program contains a 30 day cancellation period. FL, LA, ND, OK, SC, SD and TX residents: Member shall receive a full refund of membership fees, excluding registration fee, if membership is cancelled within the first 30 days after receipt of membership materials. AR and TN residents: A refund of all fees will be issued if membership is cancelled within the first 30 days. MD Residents: The membership fee and one-time registration fee (minus $5.00) will be refunded if cancelled within the first 30 days and upon return of the discount card. The range of discounts for medical or ancillary services provided under the plan will vary depending on the type of provider and medical or ancillary service received. The discount medical card program makes available, before purchase and upon request, a list of program providers, including the name, city, state, and specialty of each program provider located in the cardholder's service area.
